Join Dr. Sarah Todd to talk about how to get the most out of your options at the end of life. She’ll discuss all aspects of a MOLST form: When should you fill one out, what the different options are on the form, when to revisit the MOLST form, and the many places you need to store it. Dr. Todd will also explain the quality of life you can expect after each option listed on the form.
Understand the importance of choosing the right person as your health care proxy. Learn about what qualities make a good health care proxy and what your health care proxy should know about you and your wishes.
Dr. Sarah Todd is a board- certified emergency medicine physician who has been in practice for 20 years. She’ll share her experience and knowledge of how to have your wishes fulfilled at the end of life, how to think about and plan for these decisions, and how to communicate your wishes. She’ll also talk about how to make sure your goals of care and your communication within the medical community are consistent with each other…all in pursuit of having your care wishes fulfilled.
